 Employer's Monthly Pass Request Form

Once you have received the Employee Participation Request Forms from your employees, list the number of passes you require for each category below. You may mail, call in or fax you order to the NFTA Metro Cash Management Office. Also contact this office to arrange for pick-up, delivery and payment method (check of credit card.) Each participating company need to arrange for distribution to their participating employees.

Note: Maintain copies of your Employer's Monthly Pass Request form for tax purposes.
